Understanding Our Cash-Based Practice and How Super-bills Work


Insurance can be confusing and frustrating. At The Well Chiropractic, we believe your care shouldn’t be. That’s why we keep things straightforward and transparent.

We operate as a cash-based practice, meaning payment is made directly to our office at the time of your visit. From there, we provide you with the documentation you need if you choose to seek reimbursement from your insurance company. One of the most important tools we offer for this process is called a super-bill.

Below is a simple guide to help you understand what a super-

bill is and how to use it.

What Is a Super-bill?

A super-bill is a detailed receipt that contains the information insurance companies typically require to review an out-of-network claim. It does not guarantee reimbursement, but it gives your insurance company what they need to consider it.

A super-bill includes your personal and insurance information, our clinic’s information (name, address, tax ID, and NPI), diagnosis codes explaining why you were treated, procedure codes describing the services provided, and the amount you paid at your visit.

What Does “Cash-Based” Mean?

Being a cash-based practice does not mean we only accept cash. We gladly accept major credit cards, debit cards, and checks. Cash-based simply means we do not bill insurance companies on your behalf.

Here’s what that looks like in practice: Payment is made at the time of service, pricing is clear and discussed upfront, and there are no surprise bills weeks or months later.

This model allows us to focus on patient care instead of insurance restrictions. If you have out-of-network benefits, a super-bill allows you to submit your visit to your insurance company on your own terms.

Step 1: Request a Super-bill

Super-bills are not automatically provided at every visit. If you plan to submit your care to insurance, just let us know and we’ll prepare one for you.

Step 2: Call Your Insurance Company

Before submitting, it’s helpful to contact your insurance provider and ask a few key questions. Find out whether you have out-of-network chiropractic benefits, what percentage may be reimbursed, where your claim should be sent, and whether a claim form is required.

Having this information ahead of time can save you time and frustration.

Step 3: Complete a Claim Form

Most insurance companies require a short claim form in addition to the super-bill. These forms are usually available on the insurance company’s website or can be mailed to you upon request.

Fill out the form completely, attach your super-bill, and keep a copy for your records.

Step 4: Submit Your Claim

Send your completed claim form and super-bill using the method your insurance company prefers. Many insurers now allow online or digital submission, which can help speed up processing.

Step 5: Receive Reimbursement

Processing times vary, but most claims are reviewed within two to six weeks. If approved, reimbursement is sent directly to you rather than to our office.
