What Is Cox® Flexion-Distraction Decompression Spinal Manipulation?



If you’ve been dealing with back pain, sciatica, or a disc injury, you may have heard of Cox® Flexion-Distraction and wondered how it’s different from traditional chiropractic adjustments.

Cox® Flexion-Distraction is an evidence-based, gentle form of spinal manipulation designed to reduce pressure on spinal discs and nerves while restoring healthy movement to the spine. It is performed using a specialized table, often called a Cox Table, which allows for precise, controlled motion rather than forceful twisting or cracking.

This technique was developed by Dr. James M. Cox, DC, DACBR, and has been studied for decades in both clinical practice and research settings.


How Cox® Flexion-Distraction Works

Unlike traditional spinal manipulation, Cox® Flexion-Distraction places the spine into a gentle, measured distraction. This means the spine is carefully stretched and mobilized in a controlled way, allowing specific spinal segments to move within their normal physiological range.

Research has shown that this approach creates several important mechanical changes in the spine, particularly at the disc and nerve level.

First, the height of the intervertebral disc can increase. This is important because compressed discs often irritate nearby nerves and contribute to pain.

Second, the openings where spinal nerves exit the spine called the intervertebral foramina can increase in size by as much as 28%. This added space helps reduce nerve compression, which is a common cause of sciatica and leg pain.

Third, pressure inside the disc decreases. Normally, disc pressure can become positive and irritating when injured. Cox® Flexion-Distraction has been shown to reduce this pressure, creating a negative pressure gradient that helps relieve stress on pain-sensitive structures and spinal nerves.

Fourth, normal spinal motion is restored. Loss of movement is a key component of both acute and chronic back pain. By gently guiding specific spinal segments back into their normal ranges of motion, daily activities often become easier and less painful.

Finally, spinal manipulation stimulates the nervous system. Specialized nerve pathways respond to movement and touch, helping regulate pain, improve coordination, and support normal spinal function.


What Is the Goal of Cox® Flexion-Distraction?

The primary goal of Cox® Flexion-Distraction is to attain and maintain normal spinal motion while providing the greatest possible pain relief.

It’s important to understand that not all spinal conditions are “cured” in the traditional sense. Some conditions such as congenital spine differences, degenerative changes, or certain injuries are best managed rather than eliminated entirely. In these cases, ongoing care, proper movement habits, ergonomic training, and targeted exercises play an important role in maintaining relief.

For many patients, Cox® Flexion-Distraction offers a conservative, non-surgical option that helps control pain, improve function, and reduce the need for more invasive treatments.


A Conservative Alternative Worth Considering

Cox® Flexion-Distraction is commonly used for conditions such as disc herniation, sciatica, spinal stenosis, and chronic low back pain. Because it is gentle and highly controlled, it is often well tolerated by patients who cannot handle aggressive spinal adjustments.

For some individuals, this approach may provide meaningful relief and serve as a conservative alternative before considering injections or surgery.
